I think it has to do with whether your urethra gets fully collapsed or not. I know for me, I can piss with a semi-hard to even 3/4 hard cock. Never tried when I was at my "hardest" though. And as long as I let it stay natural and not try to bend it down, I can piss. Done this in the shower more than once.
