I definitely agree with the tshirt thing... And nothing else under it... Sometimes one of my button dress shirts is sexy and definitely a turn on.... But something else that gets me going is for her to wear a baseball cap with the hair out the back in a ponytail..... Very sexy..

There's nothing better than a surprise ;) but I'm guessing you wanted a more direct answer.
So I'd probably say a matching bra & french knickers (with a lovely see-through sheer back panel) with stockings and heels. It depends on the lady though, and the nature of our relationship, and what kind of thing we feel like doing in bed, e.g. I also love a sexy role play costume (you sometimes have to search pretty hard to find non-tacky ones!). I'm not massively into BDSM (yet!) but I really like the whole latex dominatrix look too.
mal0003 and imaginativerogue and others also made the valid point that, if you don't have much to work with, wearing nothing but a man's shirt or nothing at all can also be very sexy.
So after all that, I think what I'm trying to say is that, for me, there is no one sexiest outfit. Women are so great to look at, why restrict them to just one?

It depends on the woman, but my preference is something elegant. I'd rather have a long slip that shows and emphasizes her body rather than some Victoria's Secret she-looks-like-a-porn-star apparel. I had a Japanese girlfriend once that wore a dark unadorned kimono. It was rather severe in some ways, but it was fascinating and arousing to see her undress from it or to take it off of her, almost as if I was watching a butterfly come out of a cocoon.
