I think TonyZ is right. Mine can get entangled or something in boxers. You get the right amount of ball hang and they want to go one direction or the other and can't...yeah, it can be painful. Don't think I have enough hang to actually sit on them though.

It has happened to me but it is extremely rare. It's usually due to a tight pair of jeans and unfortunate positioning.
