The way I see, in a three/four/fivesome the action is centered on only one group of persons. For example, five participants all interacting with each other.
In an orgy, you can have many different groups of people: three persons fucking on the floor, a guy licking a girl's pussy on the sofa next to them, a girl sucking two dicks nearby, etc.
But in a way, yeah, at some point the number of participants will likely force them to split into many different small groups. I guess that when you reach around 5-6 participants, it becomes more convenient to split the action.
