Whichever of those you think is best. There is also the marketing angle of some categories attracting more views than others. Without reading it, it's a tough call. The only hard rules are around controversial categories (e.g. fantasies must be in that category). Otherwise, it's whatever you think fits the story.
As for multiple categories, that's what the tags are for. The category is the "Main subject" while tags let you know flag other subjects that appear in the story.
So my upcoming (I hope) gay male story will actually be in the category First Time (since that's what it's about), with things like gay Male, oral sex, anal sex, etc. as tags.
