Oh, no, no worries. My brother does a super excellent job of spoiling me on my birthday, every year without fail. I'm very blessed.
Well, my birthday falls on a particularly inauspicious day in American history, so I understand when people forget it. Heck, I forget that it's my birthday.

On the plus side? Every birthday since that birthday in 2001, if I wake up, turn on CNN and nothing has been bombed, hijacked, or otherwise caused horror on American soil, then my birthday is totally made. So the bar is set pretty low as to what constitutes a "happy birthday" now.

You don't mention how old your son is, but I know from very personal experience that children diagnosed with mental illness can feel very stigmatised and that this feeling can contribute to a resistance to accepting medication. What worked for me and for my son, who has depression and anxiety, was sitting him down and explaining that having a mental illness is absolutely no different from having asthma. Both are chronic diseases, and both can be well-controlled with medication--but on the flip side, left untreated, either disease can kill you.

If that's the question, the answer most likely is, probably a lot of nothing. It'd be very difficult for a person to prove that he or she (or his or her experience) was the same person(s) portrayed in a work of fiction, even if actual names were used. I'm not even sure how one would go about arguing such a thing.

That said, if you're planning to write a piece based on someone else's experience(s), and they have conveyed to you that they do not wish to have their personal experience(s) used as the basis for that piece, then you're a pretty shitty person if you go ahead and do it anyway.
